While it is a federal program it is managed by the states ,if you move from one state to another you must reapply in the new state and be approved.It was 5 months of waiting when I moved to Wisconsin from Florida if I had not had family I do not know what I would have done. The key to your application is to fill out every question on the form to the absolute fullest you can come up with. When it asks for your doctors,start with every one of them since you where 18 and make sure you know where there present records are kept so that they can easyly be found ,important if they retired. You need to be able to show you where also once quote healthy and that there has been a radical change in your health that makes it impossible for you tonremain employed as you have become an unrelieable employee due to your disability,therefore you can not remain employed as your employers can not count on you being there due to your illness. In the section for your own comments on your own symtms start with how you have to move carefully and describe it ,on getting out of bed on a bad day right up and through going to sleep again at night.Dressing ,eating cleaning(yeah right) everything childcare,driving everything .If your symtoms come and go at different intencities you must point that out so if some on takes a picture of you carrying the groceries on a good day they can't point at that and say you ae always able to do that. We tend to get dressed to go to the doctors,no where else so we took a shower,puton make-up do we always nowe don't.
